How To Refer Patients

  1. Send Paperwork
    Please complete our RX Authorization Form. Then, have your office send it to our Treatment Specialists at rx@fischermedicalsupply.com, or fax it to (805)334-3424.
  2. Eligibility & Benefits
    Once your paperwork has been received, your patient is assigned to a personal Treatment Specialist. They will create a new patient record and check into any insurance coverage. Once a determination has been made, the Treatment Specialist will make contact with your patient to review next steps.
  3. Out-of-Pocket vs. Prior-Auth
    Our Treatment Specialists can help your patient complete their order over the phone, or your patient can checkout on our website. For patients with eligible insurance coverage, we will refer their case to a partner to file the prior-authorization request. If any additional paperwork is needed – including medical letter of necessity, chart notes, etc. – a Treatment Specialist will be in touch.
  4. Ship Equipment
    Once we complete your patient’s order, we’ll ship or drop-ship via a distribution partner within 1-business day. Delivery typically takes 3-5 days.
